A Simpler Way To Plan For Dental Care
The Mountain View Smiles Loyalty Plan is designed for patients who do not have dental insurance and want a more predictable way to maintain their oral health. There are no claim forms, annual insurance maximums, or benefit approvals. You pay the annual program fee, use the included services during your 12-month benefit period, and receive plan discounts on eligible care.
This is an in-house dental care plan, not dental insurance. It can be a practical option when you expect to use routine preventive services and want savings on treatment that may come up during the year.

Included Each Year
Each active plan includes:
- 2 oral exams
- 1 emergency exam
- 2 cleanings or periodontal maintenance visits
- 2 oral cancer screenings
- 2 fluoride tooth-desensitizing treatments
- 1 set of bitewing X-rays
- Any individual X-rays needed during the year
Your clinical needs still determine which services are appropriate. Included benefits must be used during the active 12-month period and do not roll over.

Member Discounts
Active members also receive:
- 25% off additional preventive services
- 20% off usual, customary, and reasonable fees for basic services such as fillings, extractions, and root canals
- 15% off usual, customary, and reasonable fees for major services such as implants, crowns, veneers, and other eligible care
Discounts apply only to eligible dental procedures performed at Mountain View Smiles. Nitrous oxide, IV sedation, medications, products, and certain miscellaneous specialty services are excluded.

Plan fees must be paid in full before benefits become active. Fees are non-refundable, including if you cancel before the end of the benefit period.

Program Guidelines
- Your benefit period lasts 12 months from activation.
- The plan renews automatically on the anniversary date unless you provide notice at least 30 days before renewal.
- A $50 reinstatement fee applies if the plan lapses.
- The plan cannot be used with dental insurance, another dental plan, or outside financing.
- The plan does not apply to costs covered by automobile or other injury liability coverage.
- Only eligible procedures performed in this office receive plan benefits.

Is This Dental Insurance?
No. The Loyalty Plan is an annual agreement directly with Mountain View Smiles. There are no insurance claims or reimbursements.
When Do My Benefits Start?
Benefits begin after the annual program fee is paid in full and the plan is activated.
Can I Combine The Plan With Insurance Or Financing?
No. The plan may not be used with another dental plan, dental insurance, or outside financing.
Do Unused Visits Roll Over?
No. Included services must be used during the active 12-month benefit period.
Are Sedation Services Discounted?
Nitrous oxide and IV sedation are excluded from plan discounts. Medications, products, and some other specialty items are also excluded.
Can My Household Join Together?
Yes. Dual and family memberships are available for individuals living in the same household. Ask our team to review the current household terms before enrolling.
